Healthie Alternatives for Dietitians Who Bill Insurance

Healthie alternatives for insurance-billing RDs: Practice Better, SimplePractice, Kalix and Farela compared on claims, eligibility, ERAs and real monthly cost.

You chose Healthie because it looked like the grown-up option: an EHR with scheduling, charting, a client app, and insurance claims in the same place. A year in, the insurance side is where your time goes. You moved up a tier to get CMS-1500 claims, connected Claim.MD, and you still pick the codes, count the units, chase the rejections and read the ERAs yourself.

That doesn't mean Healthie is wrong for you. It means the question has changed. It's no longer "which platform has the most features". It's "which steps between the session and the payment still depend on me, and what am I paying to still do them." This guide compares the realistic alternatives on exactly that.

What Healthie does well (and when to stay)

Be fair to it first. Healthie is a real platform:

If you run a group practice, sell programs, or plan to build on an API, stay. None of the alternatives below beats Healthie at being a platform.

Why insurance-billing RDs start looking

These are the friction points we hear most, each checked against Healthie's own documentation:

  1. Billing sits behind a tier, plus an add-on. On the pricing page, the Core plan doesn't list the Claim.MD integration. It starts on Essentials ($45/month billed annually, up to 250 active clients), and the integration is listed separately as starting at $25/month plus a $50 to $100 setup fee. The AI Scribe is a usage-based add-on for Plus ($115/month billed annually) and above.
  2. Your clearinghouse account changes hands. Healthie's FAQ says an existing Claim.MD account must be transferred to Healthie for the integration to be supported, and the Claim.MD subscription is folded into your Healthie bill. That's convenient, but check how you'd get it back if you ever leave.
  3. The coding is still yours. Healthie builds the claim from what you enter. It doesn't decide 97802 vs 97803, count 15-minute units from the visit, or warn you that a Z68 BMI code can't be the primary diagnosis.
  4. Eligibility answers the wrong question. Healthie's own FAQ notes that most payers respond to the benefit type, not the procedure code, so a check rarely confirms coverage for nutrition codes by itself. You still call for visit limits and preventive versus medical routing.

Who each option fits

Practice Better fits when the reason you're leaving Healthie is cost or client experience, not billing depth. Its client portal and program tools are excellent, its Insurance Billing FAQ confirms eligibility checks, claim submission and ERA sync through your own Claim.MD account, and you keep ownership of that account. The integration is listed on every paid plan, though the $35 Starter plan caps you at 10 clients, so most insurance practices land on Professional ($69/month billed monthly). SimplePractice fits practices that share admin with therapists or other clinicians. Its claims engine is mature. Two things to know for MNT: unit-billed services default to 1 unit and you set the rest by hand, and from October 1, 2026 every electronic claim costs at least $0.39 and manual eligibility checks $0.15 on the Starter and Essential plans. Kalix is the cheapest RD-specific EHR with US claims on this list: $27 a month plus a $15 managed clearinghouse with unlimited claims and automatic remittance posting, per its help center. Farela fits a narrower case: you're a solo RD, most of your visits go to insurance, and the hours you want back are the note and the claim. Farela records the visit (video in Farela, in person or by phone), writes the structured MNT note with a PES statement and units counted from the visit time, and drafts the claim with CPT, ICD-10, modifiers and place of service. You review it and submit it through your own Claim.MD account. Eligibility runs through Claim.MD, payments are tracked from the ERA, and cash visits are invoiced through Stripe.

How to switch without losing money in flight

  1. Export first. Pull client demographics, insurance details, notes and documents out of Healthie before you cancel anything.
  2. Finish open claims where they started. Claims already submitted should be worked to payment in the old system. Timely filing limits don't pause for a migration.
  3. Sort out the clearinghouse. If your Claim.MD account was transferred to Healthie, ask both companies how to move it back. Remember that ERAs go to one clearinghouse per NPI/Tax ID, so an ERA re-enrollment can move where payments land. Our Claim.MD integration page covers enrollment.
  4. Run three real visits end to end on the new tool, from eligibility to a paid ERA, before moving everyone.

The honest bottom line

Healthie is the right answer for teams and platforms. If you're solo and insurance is your engine, the alternatives split by what you want to pay for: lower list price (Kalix), client experience (Practice Better), a shared multi-discipline system (SimplePractice), or less of the note-to-claim work landing on you (Farela). Whatever you trial, run one real claim through it and time every step you do by hand. That number is the comparison.

Which Healthie plan do I need to bill insurance?

On Healthie's pricing page (checked September 28, 2026), the Claim.MD integration is listed from the Essentials plan, shown at $45 a month billed annually with up to 250 active clients. The integration itself is listed as starting at $25 a month with a $50 to $100 setup fee. The Core plan does not list it.

Can I keep my own Claim.MD account if I use Healthie?

Healthie's Claim.MD FAQ says an existing Claim.MD account can be used, but ownership has to be transferred to Healthie for the integration to be supported, and the Claim.MD subscription is then consolidated into your Healthie bill. Plan for that before you connect, and know how you would take it back if you leave.

What is the cheapest Healthie alternative for insurance billing?

By list price, Kalix: $27 a month for its Standard plan plus a $15-a-month managed clearinghouse with unlimited claims, per Kalix's pricing and help pages in September 2026. Cheapest on paper is not always cheapest in hours, so price the manual work too.

Is Farela a full replacement for Healthie?

Not for every practice. Farela has no client portal, programs, meal plans or open API, so group practices and program-based businesses are better served by Healthie or Practice Better. It fits solo RDs whose main problem is the note and the claim after each insurance visit.

Do any of these tools confirm nutrition benefits automatically?

No tool can make a payer return what it doesn't send. Healthie's own FAQ notes that most payers answer an eligibility check with a benefit type rather than a procedure code, so a check rarely confirms coverage for 97802 or 97803 by itself. Use the electronic check for active coverage and cost share, then call for nutrition limits.
