POS 10 and POS 02 (telehealth place of service)
Place-of-service codes tell the payer where the patient was. For telehealth there are two:
- POS 10, Telehealth provided in patient's home: the patient is in a private residence (not a hospital or facility).
- POS 02, Telehealth provided other than in patient's home: the patient is somewhere else.
For Medicare professional claims, CMS instructs practitioners to use POS 02 or POS 10 for telehealth, and since January 1, 2024, Medicare pays telehealth services to patients in their homes at the non-facility rate (MLN901705; CMS Telehealth FAQ). MNT codes are on Medicare's telehealth list and registered dietitians are eligible telehealth practitioners. Through December 31, 2027, Medicare beneficiaries can receive telehealth anywhere in the US, including at home; the CMS FAQ says those location rules tighten for non-behavioral services from January 1, 2028.
Commercial payers set their own rules. Some want POS 10 with modifier 95, some want POS 02 for all telehealth, and some want the office POS with a modifier. Ask each payer and write it down.
