Weight gain brings a lot of patients to a dietitian: after a new medication, after an injury, after years of slow drift. On a claim, though, R63.5 is often the wrong code, because the condition the provider actually documented is overweight or obesity, and ICD-10-CM doesn't let you use both.

What R63.5 covers

R63.5, abnormal weight gain, is a Chapter 18 symptom code. The index sends "weight, gain (abnormal) (excessive)" and "gain in weight (abnormal) (excessive)" to it. It has two Excludes1 notes:

The R63 category also excludes bulimia NOS (F50.2-).

An Excludes1 note means the two codes aren't used together. The guidelines (Section I.A.12.a) allow an exception only when the two conditions are unrelated, and weight gain and obesity in the same patient rarely are. In practice: once the provider documents overweight or obesity, R63.5 comes off the claim.

A Z68 BMI code needs an associated reportable diagnosis documented by the provider, such as overweight or obesity. R63.5 doesn't carry its own instruction to add a BMI code, and it isn't the kind of diagnosis the guidelines give as an example. So if the patient's weight meets an overweight or obesity diagnosis the provider has documented, that's the code that carries the claim, and R63.5 isn't used.
