Pricing
Duo Practice pricing
Duo Practice is a free EHR and AI scribe for registered dietitians in the United States. The only cost is a 3.9% commission on what is collected: on insurance claims, 3.9% of what the insurer pays, so a claim paid $0 costs $0; for direct-pay clients, 3.9% of what they pay. There is no monthly fee and no card on file.
What is free
- The chart and the clients list
- Scheduling
- Session recording and transcripts (20 transcription hours a month)
- AI notes: an MNT note in ADIME form with a PES statement, drafted from the visit
- Hands, the assistant inside Duo Practice
- A one-way Practice Better client import
What billing costs
Billing is a 3.9% commission on what is collected. There is no minimum and no cap.
- Insurance claims: 3.9% of what the insurer pays on each claim. It is charged only on claims an insurer actually paid, never on one that was merely submitted, denied, or went to the deductible.
- Direct-pay clients (no insurance): also 3.9% of what is collected.
Duo drafts the claim from the visit (CPT code, units from the session length, ICD-10 codes, modifier and place of service). Nothing is sent without the dietitian's approval, and claims go out under her own NPI.
What you pay separately
Claims go through the dietitian's own Claim.MD account, which she pays Claim.MD directly (Claim.MD pricing). Billing also needs her own payer enrollments.
Refunds

What is not included
Duo Practice doesn't do prior authorizations, appeals or credentialing, and it has no programs or client app. It is built for solo and small dietitian practices that bill commercial insurance and Medicare for medical nutrition therapy.
A worked example
If an insurer pays $100 on a claim, Duo Practice's fee on it is $3.90. If the claim is denied and pays nothing, the fee is $0. A direct-pay client who pays $100 is also $3.90.
