Why Z71.3 on its own gets denied
Z71.3 means dietary counseling and surveillance. It describes the service, not the medical reason for it, so many payers want a condition next to it before they pay a 97802 or 97803.
The order that works
- The condition from the referral. Type 2 diabetes (E11), CKD (N18), prediabetes (R73.03), obesity (E66), hypertension (I10) and so on. Use the most specific code the referral and your note support.
- Z71.3, for the counseling itself.
- The BMI code (Z68.x) when weight is part of the plan. It is never first, and it needs a related diagnosis such as obesity. The BMI to Z68 finder gives the code from height and weight.
What Medicare MNT covers
Medicare Part B pays 97802 and 97803 for three conditions only: diabetes, chronic kidney disease when the patient is not on maintenance dialysis, and the 36 months after a kidney transplant. Diabetes includes gestational diabetes, but only until the pregnancy ends (the episode-of-care definition in 42 CFR 410.130). It needs a physician referral. Prediabetes, obesity, hypertension, IBS and PCOS are not covered under Medicare MNT, though many commercial plans cover them. Check the member's benefits before the first visit.
The obesity class codes
Since October 2024, ICD-10-CM has E66.811, E66.812 and E66.813 for obesity class 1, 2 and 3. Use them only when the class is written in the record; otherwise E66.9 with the Z68 code is the safer pair.
When a claim is denied anyway
Paste the code into the denial checker to see what it means. Duo Practice drafts the claim from the visit, with the diagnoses in this order, for you to review before it goes out. The chart is free; the claim service costs 3.9% of what insurers pay, with no monthly fee. See how the billing service works.
