SOAP note
A SOAP note has four parts (NCBI StatPearls: SOAP Notes):
- S, Subjective: what the patient reports: history, symptoms, eating patterns, goals.
- O, Objective: measurable data: weight, BMI, labs, vitals.
- A, Assessment: your clinical interpretation; for dietitians, this is where the nutrition diagnosis (PES statement) usually sits.
- P, Plan: interventions, education, goals and follow-up.
Referring physicians read SOAP fluently, which is why many private-practice dietitians use it for notes that get sent back to the referral source. ADIME covers the same ground organized around the Nutrition Care Process.
Neither format is required by payers. For MNT claims, what matters is content: face-to-face minutes that support the units billed, the diagnosis on the claim reflected in the assessment, the intervention delivered, and a signature with credentials. Pick one format, template it, and use it every visit so notes are fast to write and easy to audit.
