ADIME note
ADIME organizes a nutrition note around the four steps of the Nutrition Care Process (Academy: NCP overview):
- A, Assessment: food and nutrition history, anthropometrics, labs, nutrition-focused findings, client history.
- D, Diagnosis: the nutrition diagnosis written as a PES statement.
- I, Intervention: what you did and recommended, with goals.
- M/E, Monitoring and Evaluation: the indicators you will track and when you'll reassess.
Payers don't require ADIME or any other format. What a reviewer checks is whether the note supports the claim: date of service, face-to-face time that matches the units billed, the diagnosis on the claim, the intervention delivered, and a signature with credentials. ADIME tends to make that easy because the diagnosis-to-intervention chain is built into the structure.
For Medicare MNT, keep the physician's referral with the diagnosis on file alongside the notes; it is part of the record a reviewer expects (CMS Claims Processing Manual, Ch. 4, §300.2).
