Z71.3 and Z68 BMI codes: how dietitians pair them

The short answerZ71.3 (dietary counseling and surveillance) says why the patient is seeing you, and a Z68 code records their BMI, but neither one usually gets an MNT claim paid on its own. Put the medical condition the payer covers first (for example E11.9 or E66.811), then Z71.3 if your payer wants it, then the Z68 BMI code, which is never first.
Z71.3Dietary counseling and surveillance; first or secondary depending on the payer
Z68BMI; secondary only, and only with an associated diagnosis
Who documents whatBMI can come from the dietitian; the related diagnosis must come from the patient's provider
Adult BMI codesAge 20 and older: Z68.18-Z68.45
Pediatric BMI codesAge 2-19: Z68.51-Z68.56 (percentile for age)
PregnancyDon't assign Z68 codes
Since October 1, 2026Z68.1 is no longer billable; use Z68.18 or Z68.19

ICD-10-CM codes update every October 1, and the FY2027 code set took effect on October 1, 2026. Check that each code is valid for the date of service and that it matches your payer's MNT policy. For the single-code pages, see Z71.3 and the Z68 BMI code finder.

What each code does

Code Meaning Position
Z71.3 Dietary counseling and surveillance First or secondary, depending on the payer
Z68.xx Body mass index (BMI) Secondary only, never first
Condition code The medical reason for MNT (E66.x, E11.x, R73.03, E78.5, I10, N18.x…) First when the payer covers MNT by diagnosis

When the condition code must lead

Coverage for MNT is usually tied to a diagnosis. The first-listed code on the claim is the one payers check against their policy, so it should be the condition that makes the visit covered.

In the ICD-10-CM tabular list, Z71.3 tells you to use an additional code for any associated underlying condition and for BMI when known. So even when Z71.3 leads, list the condition and the BMI after it.

Common condition codes in MNT

Code Description
E11.9 Type 2 diabetes without complications
E11.65 Type 2 diabetes with hyperglycemia
R73.03 Prediabetes
E66.811 / E66.812 / E66.813 Obesity class 1 / 2 / 3 (added October 1, 2024)
E66.01 Morbid (severe) obesity due to excess calories
E66.3 Overweight
E66.9 Obesity, unspecified (payers increasingly prefer a specific code)
E78.5 Hyperlipidemia, unspecified
I10 Essential (primary) hypertension
N18.30 / N18.31 / N18.32 Chronic kidney disease stage 3 unspecified / 3a / 3b

Code to the highest specificity the referring provider documented. If the referral only says "obesity" and you need a class code, ask the provider to document the class.

Adult BMI codes (age 20 and older)

BMI Code
18.4 or less Z68.18 (from October 1, 2026)
18.5 to 19.9 Z68.19 (from October 1, 2026)
20.0 to 29.9 Z68.20 to Z68.29 (one code per whole number)
30.0 to 39.9 Z68.30 to Z68.39 (one code per whole number)
40.0 to 44.9 Z68.41
45.0 to 49.9 Z68.42
50.0 to 59.9 Z68.43
60.0 to 69.9 Z68.44
70 or greater Z68.45

Visits through September 30, 2026 still use Z68.1 (BMI 19.9 or less); from October 1, 2026 it is a header and no longer billable. Example: BMI 32.4 is Z68.32. BMI 27.0 is Z68.27.

Pediatric BMI codes (age 2 to 19)

BMI percentile for age Code
Less than 5th percentile Z68.51
5th to less than 85th Z68.52
85th to less than 95th Z68.53
95th percentile to less than 120% of the 95th Z68.54
120% to less than 140% of the 95th percentile Z68.55
140% of the 95th percentile or greater Z68.56

Z68.55 and Z68.56 were added on October 1, 2024, and Z68.54 was narrowed at the same time. If you code children, make sure your code list is current for the date of service.

Frequently asked questions

Can a Z68 BMI code be the primary diagnosis?

No. BMI codes are reported only as secondary diagnoses, and only when there is an associated reportable diagnosis such as obesity. List the condition (for example E66.811 or E11.9) or Z71.3 first, then the BMI code.

Should Z71.3 or the medical condition be listed first?

If the payer covers MNT for a specific condition (Medicare always, many commercial plans too), list that condition first. List Z71.3 first when the plan covers preventive nutrition counseling without a medical diagnosis, or when there is no qualifying condition.

Can a dietitian document BMI for coding?

Yes. The ICD-10-CM guidelines let the BMI come from other clinicians, and use the dietitian as their example, but the associated diagnosis (such as obesity) must be documented by the patient's provider.

Which BMI codes apply to children?

Patients aged 2 to 19 use the pediatric percentile codes Z68.51 to Z68.56. Adult codes are for patients 20 and older.
