Is there an AI scribe for dietitians that creates the insurance claim?

The short answerMost AI scribes stop at the note. Some general-purpose scribes, such as Freed on its Premier plan, suggest ICD-10 and CPT codes, and the AI add-ons in Practice Better, Healthie and SimplePractice write notes inside an EHR that can send claims, but in all of them you still choose the codes, count the units and build the claim. Duo Practice is built for dietitians to go from the recorded visit to the claim: it writes the MNT note, drafts the claim from it with CPT, units from the session length, ICD-10, modifier and place of service, and files it under your NPI through your own Claim.MD account once you approve. The chart and AI notes are free; billing is 3.9% of what the insurer pays. Checked October 26, 2026.

Disclosure: Duo Practice is one of the options on this page, and we wrote it. What each other tool does comes from its own public pages, checked October 26, 2026. If a vendor has added a feature since, its page wins over ours.

Is there an AI scribe that creates the claim?

Yes, but few do it. Most scribes write the note and stop. A few suggest billing codes. To get from the visit to a claim that is ready to send, the tool also has to count the units from the minutes, pair the diagnoses, fill in the provider and payer details, and connect to a clearinghouse under your NPI. For scribes judged on the note alone, see the best AI scribes for dietitians.

The table is alphabetical, not ranked. "Not described" in the codes column means the vendor's public AI pages don't mention choosing billing codes; ask in a demo. "Not described" in the claim columns means their pricing and help pages don't mention building or sending a claim. Freed and Heidi prices are as displayed on October 26, 2026.

What it takes to turn a note into an MNT claim

For medical nutrition therapy, a note becomes a claim only after five decisions:

  1. The code. 97802 for the first visit with the patient, 97803 for follow-ups, both per 15 minutes (CMS, Ch. 4 §300).
  2. The units, from the minutes in the note (units calculator).
  3. The diagnoses, in the payer's order, such as Z71.3 with a Z68 BMI code, or diabetes or kidney disease for Medicare.
  4. Place of service and modifier for telehealth.
  5. The provider details: your NPI, the referring physician on Medicare, and the payer ID.

A generic code suggestion helps with the first and third. The others need the visit length, the patient's history with you and the payer's rules. That is why a scribe that suggests codes still leaves a claim to build, and why the note-to-claim gap is where insurance practices lose the most time.

How Duo Practice goes from recording to claim

Duo writes the MNT note from the recording, drafts the claim from it, checks it against payer rules it knows, and files it under your NPI through your own Claim.MD account once you approve. The step-by-step flow is on the AI scribe that files the claim.

The chart and 20 transcription hours a month are free. Billing is 3.9% of what the insurer pays on each claim, and a denied claim costs nothing. It doesn't do prior authorizations, appeals or credentialing, and it has no client app or programs.

We recorded the same 37-minute visit in Practice Better with its AI summary and in Duo Practice. Practice Better had the chart and claim ready in 6 minutes 35 seconds, Duo in 1 minute 56. It is one visit, recorded by us, so read it as a demo and not a benchmark. Watch it with a full text transcript, or read how the AI scribe files the claim.

When a note-only scribe is the better pick

If your clients pay cash, you don't need a claim at all, and a scribe that writes good notes is enough. If you already live in Practice Better, its AI add-on at $10 a month keeps everything in one tool. If you need a scribe for many disciplines across a group, a general-purpose scribe covers more templates than an MNT-focused one. Our AI scribe checklist for dietitians has a 20-minute trial to run on any of them.

Duo Practice, in short

What it is
Duo Practice is a free EHR with insurance billing built in. It drafts the note and the claim from the visit, files the claim under your NPI once you approve it, matches the payment and explains denials.
Who it's for
Independent registered dietitians who bill insurance.
Price
The chart is free. Billing is 3.9% of what the insurer pays on each claim, with no monthly fee, so a claim paid $0 costs $0. You pay Claim.MD directly for your own clearinghouse plan.
How it differs
Practice Better, Healthie and SimplePractice give you claim tools and leave the claim work to you. Duo Practice does that work, and it can run on top of the EHR you already use.
The insurance claim section of a visit in Duo Practice, with CPT 97802, diagnosis Z71.3, session minutes and units.
The claim for a test visit in Duo Practice: CPT 97802, Z71.3, and units worked out from the session's minutes. You check it against the chart and press Submit.

Frequently asked questions

Does Duo Practice send claims without my approval?

No. Duo drafts the claim and you approve it before it goes to your Claim.MD account. If Transmit Approval Required is on in Claim.MD, the claim also waits for approval there.

Can I trust AI-suggested CPT codes for MNT?

Check them. For MNT the code depends on whether this is the first visit with the patient (97802) or a follow-up (97803), and the units depend on the minutes in the note. Review every suggested code and unit count before the claim goes out, whichever tool suggested it.

Do I need a business associate agreement for an AI scribe?

Yes, if the tool handles patient audio or notes. Ask each vendor for its BAA before you record a real visit, and get the client's consent to record under your state's rules.
